How to Submit Your Personalized Recovery Plan

Once you’ve completed your recovery plan—whether you filled it out digitally or wrote it by hand—you’re ready to submit it and receive your Certificate of Completion.

Here’s how you can upload your completed plan:

If You Completed Your Recovery Plan on a Computer:

Save your completed Recovery Plan Template as a PDF or Word document. When you reach the upload section, simply attach your file and submit.

If You Wrote Your Recovery Plan by Hand:

You can submit it one of three easy ways:

  1. Take Photos of Each Page:
    Use your phone or tablet to take clear, well-lit pictures of each page of your handwritten plan. Make sure all the writing is visible and easy to read. Then, upload each image when prompted during the submission process.

  2. Scan Your Pages into a PDF:

    If you have access to a scanner, you can scan your handwritten pages and save them as a single PDF document. This is a great option if you want to keep a digital copy for yourself as well.

  3. Use a Mobile Scanning App:

    There are free apps available like Adobe Scan, CamScanner, or Genius Scan that allow you to take photos with your phone and automatically convert them into a clean, easy-to-read PDF. Just scan, save the file, and upload it with your submission.
